Job Summary

The Service Coordinator assumes responsibility for coordinating programs and services to help residents maintain a good quality of life and age in place. This role involves assisting in assessing and identifying resident and family needs, as well as outside resources to meet those needs.

Main Responsibilities
  • Responsible for networking with community organizations to offer residents affordable and new services.
  • Initiates and completes psychosocial assessments, interviewing patients, families and others to evaluate current condition and identify potential adjustments.
  • Provides general case management and referral services, maintaining complete and updated resident files.
  • Covers various areas such as physical, mental, psychological and/or social aspects related to aging and moving in/moving out.

PVM is a senior living community and services faith-based non-profit organization inviting a dynamic individual to join our team as Part-Time Service Coordinator at the Village of Warren Glenn located in Warren, MI.

Requirements

To be successful in this role, candidates should have:

  • A minimum degree in social work, psychology or gerontology preferred.
  • Two or more years of experience in a social service delivery with elderly and/or family population.
  • Demonstrated working knowledge of supportive services and other resources in the area served by the project.
  • Excellent communication, comprehension and interpersonal skills.
  • Working knowledge of Windows, Microsoft Office Suite and resident software data systems (AASC online, Care Guide, RealPage or similar related systems).
Compensation

The estimated annual salary for this position is around $60,000-$80,000 based on national averages for similar roles in the region.
