Job Summary

This role involves providing skilled nursing care to patients in their homes, collaborating with our interdisciplinary team to develop and implement personalized care plans. You will be responsible for performing patient assessments, making referrals as needed, and teaching patients self-care techniques to promote independence and overall well-being.

Key Responsibilities:
  • Conduct thorough patient assessments and create comprehensive care plans in collaboration with the care team.
  • Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.
  • Promotes patient health and independence through education and rehabilitative measures.
  • Supervises LPNs and HHAs to ensure seamless care delivery.
  • Completes timely and accurate documentation of patient progress.
  • Communicates effectively with the clinical manager and care team regarding patient status and progress.
  • Participates in ongoing education and professional development to enhance clinical skills and knowledge.
Requirements:
  • 1+ year of experience as a Registered Nurse (RN) in a clinical setting.
  • Current RN license, specific to the state(s) assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
