We are seeking an experienced Registered Nurse to join our team as an RN Care Manager.

This exciting opportunity offers a challenging and rewarding career path for a skilled RN who wants to make a meaningful difference in patients' lives. As an RN Care Manager, you will play a crucial role in ensuring that patients receive the best possible care throughout their hospital stay.

Main Responsibilities:

  • Collaborate with patients, families, physicians, and interdisciplinary teams to develop individualized care plans
  • Facilitate patient navigation through the Emergency Department and acute care episode, promoting efficient and cost-effective care
  • Ensure seamless transitions between levels of care, including discharge planning and post-acute services
  • Work closely with Social Workers and other staff members to address complex psychosocial needs and barriers to care
  • Provide education and coaching to patients, families, and medical staff on evidence-based protocols and interventions
  • Participate in performance improvement initiatives and contribute to departmental quality reporting

Requirements:

  • Registered Nurse licensure in the State of Montana
  • Minimum 3-5 years of acute care experience
  • Baccalaureate degree or equivalent experience preferred
  • Certification in Case Management and/or Utilization Review desired

What We Offer:

The Staff Pad offers a competitive salary range of $85,000 - $110,000 per year, depending on experience, plus a comprehensive benefits package, including medical, dental, and vision insurance, retirement plan, and paid time off.
