Job Description

We are seeking a highly skilled and dedicated Bilingual Registered Nurse Care Coordinator to join our team at The Staff Pad in Helena, Montana.

Responsibilities
  • Collaborate with patients, families, physicians, and interdisciplinary teams to facilitate efficient and cost-effective care.
  • Assure and monitor the level of care from the Emergency Room through discharge.
  • Initiate evidence-based protocols and order sets according to patient admission diagnosis.
  • Influence treatment plan progression, facilitate patient navigation through acute care episodes, and propose transition plans within 24 hours of admission.
  • Serve as an advocate for patients and families throughout the ED experience or entire acute episode of care.
Requirements
  • Minimum of 3-5 years of acute care experience.
  • Strong organizational and time management skills.
  • Ability to work independently and collaboratively as part of a self-directed Care Management work team.
  • Demonstrated competence in performing all areas included in the Care Management/UR RN job role.
Estimated Salary

$85,000 - $110,000 per year, based on location and experience.
