About the Role:
We are seeking a dedicated and compassionate professional to fill the position of Team Case Manager. This role offers the opportunity to make a positive impact on the lives of adults with intellectual disabilities living in community-based settings.

In this role, you will manage a caseload of 7-10 individuals, providing case management support and ensuring that all necessary reports and documentation are completed accurately and efficiently. You will also maintain electronic health care records for each individual assigned to your caseload, upload data into the EHR system, and participate in On-Call responsibilities for assigned RRSI Director, Team, and respond to individual care needs and or critical staffing issues as needed.

You will collaborate with Program Supervisors/Directors to enhance the quality of care through individual choice in accordance with the agency's mission. You will also communicate and advocate for clients in accordance with RRSI Code of Ethics and all applicable DBHDS and Human Rights regulations.

Key Responsibilities:
  • Manage a caseload of 7-10 individuals to complete quarterly reports, plan updates, fall risk plans/assessments, annual paperwork, support instructions, and assessment updates as needed.
  • Maintain the electronic health care record for individuals assigned to ensure that all data is uploaded into the EHR system.
  • Participate in On-Call responsibilities for assigned RRSI Director, Team, and respond to individual care needs and or critical staffing issues as needed.

Requirements:
  • Bachelor's Degree in Human Services preferred, or a field related to community services.
  • One or more years of directly related experience or any combination of training and experience which provides the required knowledge, skills, and abilities.
  • Valid Virginia Driver's license and acceptable Department of Motor Vehicles Driving record and dependable transportation.
