Job Title: Admissions Registered Nurse

We are seeking a skilled and compassionate Admissions Registered Nurse to join our team at Boro Park Center for Rehabilitation and Healthcare. As an Admissions Registered Nurse, you will play a vital role in ensuring the smooth transition of new residents into our facility.

Key Responsibilities:
  • Conduct thorough admission assessments to identify residents' needs and develop individualized care plans.
  • Collaborate with the interdisciplinary team to coordinate the admission process and ensure seamless transitions.
  • Review and evaluate residents' care plans to ensure compliance with facility standards and regulatory requirements.
  • Maintain accurate and up-to-date records, including MDS and care plans.
  • Participate in discharge planning and coordinate with the interdisciplinary team to ensure a smooth transition of care.
Requirements:
  • Active and unrestricted NYS Registered Nurse License.
  • Prior experience as an admissions nurse in LTC settings.
  • Excellent clinical assessment skills and ability to make independent decisions.
  • Strong leadership and supervisory skills, with the ability to direct and supervise resident care as needed.
  • Computer proficiency in MS Office (Excel, Word, Outlook).
