The Role:

  • Partner with external providers to enhance quality of care delivered to members
  • Manage multiple complex initiatives impacting healthcare delivery and services
  • Collaborate with external facing and plan quality improvement leaders

You'll be responsible for:

  1. Providing post-discharge care coordination and management services
  2. Developing individualized care plans through clinical assessment and education
  3. Working collaboratively with internal teams and external partners

The Ideal Candidate:

  • RN License (California)
  • Utilization and coordination of care management experience
  • Epic Experience preferred
