About the Role

We are seeking a skilled and compassionate Nurse Navigator - RN to join our team at Dartmouth Hitchcock Medical Center in Lebanon, NH. As a key member of our multidisciplinary team, you will play a vital role in providing care coordination, education, and support to patients, families, and caregivers throughout the care continuum.

Responsibilities
  • Assess patient needs and develop a holistic plan of care to address unmet needs and facilitate referrals to appropriate services.
  • Identify and mitigate barriers to care, such as transportation, childcare, and financial constraints.
  • Collaborate with healthcare providers, social workers, and other team members to ensure seamless care coordination.
  • Provide education and support to patients, families, and caregivers to promote understanding of the cancer healthcare system and available resources.
  • Participate in the tracing of metrics and patient outcomes to evaluate the effectiveness of the navigation program.
Qualifications
  • Graduate from an accredited Nursing Program required.
  • Bachelor of Science Degree in Nursing (BSN) required; Master's preferred.
  • Three years of oncology experience in an acute or ambulatory care setting preferred.
  • Strong organizational skills and ability to prioritize and reprioritize.
  • Ability to develop collaborative relationships internally and externally.
