Job Summary

The Hospice Care Navigator is a critical role that requires a unique blend of sales, marketing, and patient advocacy skills. As a Hospice Care Navigator, you will be responsible for developing and executing a marketing and sales plan to support the growth of our hospice services. This will involve building relationships with key referral contacts, managing leads, and educating the public about the benefits of hospice care.

Key Responsibilities

  • Develop and Manage the Contact and Leads Database

Develop and maintain a comprehensive database of contacts and leads, including healthcare professionals, community organizations, and other stakeholders.

Establish Relationships with Key Referral Contacts

Build and maintain relationships with key referral contacts, including doctors, nurses, social workers, and other healthcare professionals.

Drive On-Campus or On-Location Appointments

Make outbound calls and conduct appointments to drive sales and enrollment in hospice care.

Develop and Execute a Marketing and Sales Plan

Develop and execute a comprehensive marketing and sales plan to support the growth of our hospice services.

Track and Report on Referral Sources

Track and report on the source of referrals and adjust the sales plan accordingly.

Educate the Public about Hospice Care

Educate the public about the benefits of hospice care, including end-of-life care, advance directives, and hospice eligibility.

Qualifications

  • Education and Experience

A Bachelor's degree in Healthcare, Social Work, or Marketing is preferred, or equivalent experience. A valid state of Ohio RN or LPN license is also required.

Skills and Abilities

Strong interpersonal skills, excellent communication skills, and the ability to work with older adults and their families.

Other Requirements

A valid Ohio driver's license and a strong understanding of hospice care and end-of-life issues.
