Post-Acute Liaison Specialist

We are seeking a highly skilled Post-Acute Liaison to join our team at The Terraces of Los Gatos - a HumanGood community. As a key member of our team, you will be responsible for generating business from referrals sources to meet our community's revenue and census goals.
Key Responsibilities:- Develop relationships with community hospitals, physician offices, and other community outreach programs to drive business growth.
- Coordinate external professional relationships to achieve the highest possible occupancy within budgetary and regulatory guidelines.
- Collaborate with admission coordinators and other key team members to ensure efficient referrals and care transitions.
- Engage with collaborative network leadership, risk holding physicians, and managed care partners to share insights and drive program enhancement opportunities.
- A competitive salary range of $61,680.73 to $92,000, with a performance-based bonus.
- A comprehensive benefits package, including 20 days of paid time off, 7 company holidays, 401(k) with up to 4% employer match, and more.
