Key Responsibilities

As a Patient Care Coordinator, you will be responsible for:

  • Providing personalized care to elderly, convalescent, or disabled individuals
  • Monitoring and reporting changes in patient health status
  • Assisting with daily living activities, such as bathing and dressing
  • Administering medication and changing bandages as needed
  • Teaching family members effective patient care techniques

Estimated salary: $18.50 - $23.25 per hour based on location and experience.

Requirements

To succeed in this role, you must have:

  • A valid CHHA certification
  • Experience in hospice, home health, or related fields
  • A positive and patient demeanor
