We are seeking a dedicated Care Coordinator Nurse to join our Home Health team in Boise. As a key member of our team, you will provide exceptional care to patients in the comfort of their own homes. Responsibilities
- Develop and implement comprehensive care plans tailored to each patient's specific needs

- Collaborate with healthcare professionals to ensure seamless communication and coordination of care

Requirements
- ASN or BSN in Nursing
- At least 1 year of RN experience, preferably in home health or acute care
- Current RN license in the state of Idaho and Basic Life Support (BLS) certification
