Job Overview:

Core Responsibilities
The main objective of this position is to deliver consistent daily nursing assistance and services to each assigned resident, adhering to their individual assessments and care plans, while following directives from supervisory staff.

Key Responsibilities
  • Accurately identify residents using wristbands or photo identification prior to administering treatments or serving meals.
  • Utilize only approved abbreviations when documenting information.
  • Promptly report any changes in a resident's condition to the Nurse Supervisor/Charge Nurse.
  • Maintain thorough and descriptive records on flow sheets, notes, and charts.
  • Document any accidents or incidents observed during the shift.
  • Uphold the confidentiality of residents' protected health information and report any suspected breaches to the Administrator.
  • Notify supervisors of any unauthorized attempts to access the facility's information systems.
  • Execute only those nursing procedures for which you have received training.
  • Prepare the resident's room for their arrival, ensuring it is clean and welcoming.
  • Welcome residents and assist them to their designated rooms.
  • Introduce residents to their roommates and other staff as appropriate.
  • Ensure residents are comfortable upon arrival.
  • Catalog and label residents' personal belongings as directed.
  • Assist residents with packing personal items during transfers or discharges.
  • Transport residents to new locations within the facility as needed.
  • Aid in loading and unloading residents from vehicles when necessary.
  • Foster a warm and inviting atmosphere, promoting a calm environment throughout the unit.
  • Collaborate with nursing staff during scheduled meetings to identify and address service improvement areas.
  • Document and address any complaints or grievances from residents.
  • Participate in nursing reports as instructed.
  • Adhere to established protocols regarding exposure to blood and body fluids.
  • Assist with bed-making and additional comfort measures as requested.
  • Measure and record vital signs, including temperatures, pulses, and respirations, as instructed.
  • Ensure residents who cannot call for assistance are checked frequently.
  • Respond promptly to resident calls for help.
  • Regularly check on residents to ensure their personal care needs are met.
  • Assist residents with food arrangements, providing necessary information about their meals.
  • Keep track of residents' food and fluid intake, reporting any changes to the Nurse Supervisor/Charge Nurse.
  • Maintain cleanliness and availability of residents' water pitchers.
  • Provide post-meal care, including cleaning residents and their dining areas.
  • Follow established policies and procedures in all assigned tasks.
  • Work collaboratively with other departments to ensure comprehensive nursing services.
  • Communicate any lateness or absences to the facility.
  • Report any occupational exposures to blood or hazardous materials as per facility guidelines.
  • Assist residents with daily personal care tasks, including hygiene and dressing.
  • Ensure residents remain dry and comfortable by changing soiled clothing and linens.
  • Support residents in preparing for medical tests and procedures.
  • Assist with the physical movement and positioning of residents as needed.
  • Implement restorative and rehabilitative procedures as directed.
  • Prepare residents for physical examinations.
  • Monitor residents for personal care needs, ensuring they are met according to their preferences.
  • Observe and report any signs of skin breakdown or pressure sores.
  • Report any unexplained injuries or skin issues.
  • Monitor disoriented or comatose residents, documenting observations as required.
  • Execute special treatments as instructed.
  • Immediately inform the Nurse Supervisor/Charge Nurse of any missing residents.
  • Use only equipment you are trained to operate.
  • Report any malfunctioning equipment to the Nurse Supervisor/Charge Nurse.
  • Maintain confidentiality regarding all resident care information.
  • Report any allegations of resident abuse or property misappropriation.
  • Respect residents' rights to refuse treatment and report such requests to your supervisor.
Supervisory Responsibilities
This role does not include supervisory duties.

Qualifications

Education and Experience
Minimum requirement of a 10th-grade education.

Language Proficiency
Ability to read and interpret various documents, including business publications and regulatory guidelines.
Ability to write reports and correspondence effectively.

Mathematical Skills
Ability to apply mathematical concepts in practical situations.

Reasoning Skills
Ability to solve practical problems and interpret a variety of instructions.

Licenses and Certifications
Must hold a valid Certified Nursing Assistant license in accordance with state regulations.
CPR certification preferred.
