Job Summary

We are seeking an experienced and reliable Physical Therapist to join our team at Valence Care Family of Home Care Companies. As a Physical Therapist, you will conduct home visits in Suffolk County, providing physical therapy services to patients according to a written physician's plan of care.

Key Responsibilities
  • Evaluate home environments to assess patient needs
  • Assess muscle strength, mobility, gait, and range of motion to determine potential for rehabilitation
  • Direct physical therapy treatment and instruct patients and families/caregivers in the use and care of therapeutic appliances
  • Determine priority needs for physical therapy and report patient reactions to treatment or changes in condition to physicians
  • Initiate physical therapy programs and instruct other personnel and/or family/caregiver members in certain phases of physical therapy
Requirements
  • Possess a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization
  • Licensed to practice as a physical therapist within the State, if applicable
  • Two years of appropriate experience as a physical therapist, with community/home health experience preferred
  • Demonstrate good verbal and written communication and organization skills
  • Possess and maintain current CPR certification
