About the RoleWe are looking for a dedicated and compassionate Occupational Therapist to provide high-quality care to our patients in their homes. As an Occupational Therapist with VNA Health Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities- Assess and evaluate patients for occupational therapy needs and develop personalized plans of action.
- Provide occupational therapy services to patients in their homes, promoting independence and functional ability.
- Collaborate with healthcare teams to ensure comprehensive care and coordinate services.
- Document patient progress and outcomes, updating care plans as needed.
- Identify counseling needs and develop teaching strategies to support patient education and empowerment.
- Occupational Therapy Licensure in NJ with AOTA certification required.
- 2-3 years of prior clinical experience and community-based experience preferred.
- Strong computer skills and effective communication and interpersonal skills.
