Job Summary

We are seeking a highly organized and detail-oriented Patient Care Coordinator to join our team at Amedisys. As a Patient Care Coordinator, you will play a critical role in ensuring the smooth operation of our care centers by coordinating patient care services, scheduling appointments, and communicating with patients, clinicians, and other stakeholders.

Key Responsibilities
  • Schedule Patient Visits: Ensure that patient assessment visits, including Oasis visits, are scheduled and performed in a timely manner.
  • Manage Clinician Schedules: Prepare weekly clinician schedules and review them daily to determine capacity opportunities and ensure productivity standards are being met.
  • Coordinate Patient Care: Work collaboratively with field clinicians to ensure all patient visits are scheduled and completed as ordered and within the care center.
  • Communicate with Patients and Clinicians: Manage calls from patients and field staff related to scheduling issues and refer clinical and performance-related issues to the Clinical Manager.
  • Monitor Hospitalized Patients: Communicate and ensure the team follows up as necessary.
  • Complete Schedules: Ensure all orders and services are included as required.
  • Support Clinical Manager: Work collaboratively to ensure timely communication with patients, clinicians, referral sources, BD team, and other office personnel.
  • Run Reports and Respond to Workflow: Run all applicable reports and respond to workflow, taking appropriate actions.
  • Assist with Patient Transfers: Assist with internal or external transfer of patients between care centers and/or Hospice services.
  • Perform Other Duties: Perform other duties as assigned.
Qualifications
  • Current License: Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse specific to that state the employee is assigned to work by the company.
  • Education: High School diploma or GED equivalent.
  • Experience: One (1+) year of administrative experience in a healthcare environment.
  • Preferred Qualifications: One (1+) year of scheduling experience in a healthcare environment.
