Job Summary:

We are seeking a skilled Physical Therapy Assistant to join our team at Eden Health. As a Physical Therapy Assistant, you will play a vital role in providing high-quality patient care and contributing to the success of our home health care services.

Key Responsibilities:
  • Implement treatment plans ordered by physicians to patients on caseload, ensuring effective and efficient care.
  • Review caseload with supervising physical therapists on a regular basis, adhering to best practices and state guidelines.
  • Evaluate assessment findings and communicate relevant information to other healthcare team members in a timely manner.
  • Guarantee that changes in patients' clinical status are communicated to other healthcare team members, necessary office staff, and physicians as per Agency policy.
  • Provide education to patients, their families/caregivers, and other staff as appropriate, in physical therapy modalities.
  • Deliver physical therapy modalities in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Participate in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
  • Identify risk of injuries or threats to life and health in the patient's environment and implement appropriate interventions to reduce those risks.
  • Maintain current knowledge of community resources and identify and access appropriate external and internal resources to meet patient and/or family needs.
  • Complete accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion.
Requirements:
  • Licensed, registered, or certified as a physical therapist assistant, if applicable, by the state in which practicing, unless licensure does not apply and meets one of the following requirements:
  • Graduated from a physical therapist assistant curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association;
  • Or if educated outside the United States or trained in the United States military, graduated from an education program determined to be substantially equivalent to physical therapist assistant entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association and;
  • Passed a national examination for physical therapist assistants.
Preferred Qualifications:
  • Prior home health experience;
  • Valid driver's license and reliable transportation;
  • Proof of automobile insurance (MVR checked);
  • Proof of current CPR certification;
  • Able to pass criminal background check and national sex offender clearance.

Pay Range:

$105,000 - $115,000 per year
