Patient Transfer Coordinator

We are seeking a highly organized and detail-oriented Patient Transfer Coordinator to join our team at UNC Health Care. As a key member of our Transfer Center, you will play a critical role in ensuring the smooth and efficient transfer of patients between healthcare facilities.
Key Responsibilities- Monitor and triage patient transfer requests, conducting assessments and facilitating provider consultations to develop transfer plans.
- Collaborate with clinical teams to coordinate patient transfers, ensuring seamless communication and effective logistics management.
- Provide exceptional customer service, identifying and escalating barriers to efficient transfer operations.
- Assess the clinical appropriateness of proposed transfers, using established medical necessity criteria and departmental guidelines.
- Document activities, events, and information in established software systems, ensuring accuracy and compliance with departmental standards.
- Associate's degree or diploma in nursing.
- Three years of clinical experience required.
- Must be licensed to practice as a registered nurse in the state of North Carolina.
