About the Role

We are seeking a highly skilled Licensed Clinical Social Worker to join our team in the Palliative Care Department. As a key member of our team, you will provide intensive psychosocial support services to palliative care patients and their family members.

Responsibilities
  • Provide individual and group therapy sessions to patients and their families
  • Develop and implement treatment plans to address patients' physical, emotional, and social needs
  • Collaborate with healthcare teams to ensure comprehensive care
  • Conduct assessments and evaluations to identify patients' needs and develop plans to address them
  • Document patient progress and maintain accurate records
Requirements
  • LCSW license
  • Ability to obtain ACHP-SW Certification (Advanced Certified Hospice & Palliative Social Worker) within two years of employment
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively

  • Full-time position with 40 hours per week
  • Monday through Friday, 8am to 5pm in our office
  • Competitive salary range: $65,000 - $85,000 per year
