We are seeking a highly skilled Licensed Clinical Social Worker to join our team in the Palliative Care Department. As a key member of our team, you will provide intensive psychosocial support services to palliative care patients and their family members.
Responsibilities- Provide individual and group therapy sessions to patients and their families
- Develop and implement treatment plans to address patients' physical, emotional, and social needs
- Collaborate with healthcare teams to ensure comprehensive care
- Conduct assessments and evaluations to identify patients' needs and develop plans to address them
- Document patient progress and maintain accurate records
- LCSW license
- Ability to obtain ACHP-SW Certification (Advanced Certified Hospice & Palliative Social Worker) within two years of employment
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Full-time position with 40 hours per week
- Monday through Friday, 8am to 5pm in our office
- Competitive salary range: $65,000 - $85,000 per year
