We are seeking a skilled RN Care Manager to join our team at Providence Medical Group. As a RN Care Manager, you will play a critical role in providing high-quality, patient-centered care to our most vulnerable patients in their homes.
Job SummaryThe RN Care Manager will be responsible for coordinating care for patients with complex medical needs, including those with chronic conditions, advanced medical needs, and life-threatening conditions. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

- Develop and implement individualized care plans that meet the unique needs of each patient.
- Work closely with our interdisciplinary team to ensure seamless care coordination and communication.
- Provide education and support to patients and their families to promote self-management and independence.
- Collaborate with community resources and healthcare providers to ensure comprehensive care.
- 2 years of clinical experience in medical-surgical, acute care, or long-term care.
- 1 year of experience in community-based or ambulatory care.
- 1 year of experience in care of the frail elderly.
- Oregon Registered Nurse License upon hire.
- Bachelor's Degree in Nursing.
- Home health, hospice, palliative care, managed care, or skilled nursing facility background.
- 1 year of care management experience with diverse patient populations.
