Your primary responsibilities will include triaging and collaborating with outside healthcare providers and Mental Health Partners staff to ensure continuity of psychiatric care.

Key Responsibilities
  • Ensure continuity of psychiatric care through provisions of services, including discharge and referral
  • Triage healthcare situations to determine the needed level of intervention in crisis situations
  • Verify and carry out prescriber orders
  • Record changes in medications, laboratory tests, and prescriber recommendations
  • Communicate current psychiatric medication orders, missed medications, or other pertinent psychiatric information with clients, prescribers, and nursing staff, clinicians, and family members as appropriate
  • Administer medications, including injections, and educate clients regarding medications (risks, benefits, and side effects and medication compliance)
  • Maintain thorough, accurate, and timely documentation of the provision of services, assessments, evaluations, and progress toward the completion of clients' treatment goals consistent with clinical and administrative policies and procedures
  • Facilitate the completion of medical tests or re-tests as referred or ordered, including drawing blood, obtaining urine, and running basic lab tests on portable lab devices
  • Act as liaison between the prescribers and outside healthcare providers, including transfer of ongoing medical or psychiatric care to PCPs
  • Maintain necessary medical supplies
  • Provide nursing support, guidance, and direction to clinical staff and volunteers
  • Oversee the administration of medications by certified staff and provide training as indicated
  • Participate in team meetings
  • Participate in client groups
  • Meet defined individual and department goals, activity metrics, and Key Performance Indicators
  • Promote, support, and demonstrate through both behavior and job performance and adhere to policies, procedures, on a day-to-day basis
  • Effectively interact and communicate with other MHP staff/clients/customers/partners/etc.
  • Regular and consistent attendance is required to perform other essential functions of the job
Specialized Duties
  • May conduct wellness-related classes for clients and/or staff
  • Mobile Team Meet will clients in their homes or in the community to deliver medications, give injections, or any other medication management
  • Collaborate with partners and other medical professionals associated with MHP client care
  • Will require reliable transportation to drive to/from client meetings
  • Warner House May include holiday, night, and/or weekend shifts
  • Walk-In Crisis Includes holiday, night, and/or weekend shifts
  • Provides consultation to walk-in crisis and Adult Respite admissions
  • Provides basic medical clearance for admission to Crisis Stabilization Unit (CSU), Adult Respite, and other higher levels of care where appropriate
Requirements
  • Bachelor's of Science in Nursing degree strongly preferred
  • RN licensed to practice in the state of Colorado required
  • Current CPR card required
  • At least two years relevant work experience required
  • Walk-In Crisis team - at least three years relevant work experience required
About Mental Health Partners

Mental Health Partners began in 1962 as a clinic, received non-profit status in 1964, and was federally designated as a comprehensive community mental health center in 1971. Since then, we have expanded to meet the growing needs of the community in both counties, with locations in Boulder, Broomfield, and Longmont. We are the only agency in Boulder and Broomfield counties that provides comprehensive psychiatric services to county residents regardless of their ability to pay.

People served by Mental Health Partners generally do not have access to services in the private sector because they cannot afford to pay the significant fees, and most do not have insurance.

Mental Health Partners has 24-hour emergency psychiatric services and outpatient offices for infants, children, adolescents, adults, and families. Many programs and services are available, including school-based programs, substance abuse treatment, rape crisis counseling, advocacy, and education. Mental Health Partners offers bilingual/bicultural services and an in-house pharmacy.

It is estimated that of the 360,000 residents of Boulder and Broomfield counties, over 60,000 individuals suffer with a form of mental illness. This number is only compounded when family, friends, and co-workers are affected by the disabling conditions of a person who is struggling with a mental illness. Annually, Mental Health Partners serves over 17,000 individuals and our Emergency Psychiatric Services Team responds to 10,000 calls. We serve more women than men and thirty-six percent of our clients are multicultural.
