About Our Opportunity

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our HouseCalls team. As a Care Coordinator - Gerontology Specialist, you will play a vital role in delivering preventive care services to patients in need.

What You'll Do
  • Conduct in-home assessments to identify acute issues and connect members to key support services
  • Perform evidence-based practice screenings to reduce health risks and improve outcomes
  • Collaborate with a multidisciplinary team to develop comprehensive care plans that address patients' physical, mental, and social needs
Your Qualifications

To succeed in this role, you will need:

  • A master's degree in nursing (MSN) or a doctoral degree in nursing (DNP) from an accredited program
  • Licensure as a Registered Nurse (RN) or a Nurse Practitioner (NP)
  • Active ANCC or AANP national certification as a NP
  • Experience working in gerontology, cardiology, internal medicine, or endocrinology

New York Residents Only: This role is located in New York and requires travel throughout the state. Ability to travel up to 5% of the time is required. The estimated salary for this role is $133,400 annually.
