Position Overview

Key Responsibilities:

  • Facilitate service coordination for individuals with Autism and Developmental Disabilities
  • Assist clients in navigating available supports and services
  • Engage collaboratively with clients and team members
  • Design personalized support plans
  • Oversee adherence to plans and the overall welfare of clients
  • Maintain accurate documentation of services provided
  • Inform clients about community service options
  • Organize meetings to address individual needs
