We are seeking a skilled Clinical Coordination Specialist to join our team at Actalent. As a Clinical Coordination Specialist, you will play a critical role in coordinating referrals, verifying and entering physician's orders, identifying the level of care required by patients, reviewing documentation to determine facility placement, coordinating orders with pharmacists, and working closely with referring hospitals, physicians, skilled nursing facilities, patients, and families.
Responsibilities:
- Coordinate referrals and verify physician's orders
- Identify the level of care required by patients and review documentation to determine facility placement
- Coordinate orders with pharmacists and work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families
- Work in an office setting with no hands-on patient care
Requirements:
- LPN or RN license in Utah
- At least 1 year of experience in a skilled nursing facility
- BLS certification
Work Environment:
- M-F schedule, 10am-6pm
- Will work a weekend every 6-8 weeks (days off will be adjusted accordingly)
- Will be on call every 6-8 weeks
- Business casual dress code
