We are seeking a skilled RN Case Manager to join our team at Professional Case Management. As a RN Case Manager, you will be responsible for conducting in-home nursing visits for our clients, providing case management services, and directing a team of RNs, LPNs, and caregivers in the delivery of care.
Key Responsibilities:- Conduct in-home nursing visits for clients, including physical assessments, medication management, and home safety evaluations.
- Provide case management services to assigned clients, including developing care plans and coordinating with healthcare providers.
- Direct a team of RNs, LPNs, and caregivers in the delivery of care, ensuring that clients receive high-quality, patient-centered care.
- Collaborate with regional directors and other personnel to identify and correct issues, and to improve services.
- Graduate of a state-approved school of professional registered nursing.
- BSN preferred.
- Current, unrestricted RN license in the state(s) of practice.
- Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field.
- One (1) year of supervisory and/or case management experience preferred.
- Current CPR certification.
- Competitive hourly rate of $42.00, plus shift differentials where applicable.
