Patient Care Assistant

Position Overview:
Under general supervision, the Patient Care Assistant is tasked with delivering essential aspects of patient care while working under the guidance of a Registered Nurse.
Care Plan:
The specific care requirements and visit frequency are outlined in the Patient Care Plan.
Qualifications:
- Education: High School diploma or GED.
- Licensure: Current registration as a Certified Nursing Assistant with the Maryland State Board of Examiners of Nurses. Must possess a valid driver's license and have an insured automobile in good working condition.
- Experience: Prior experience in an acute care or home care environment is preferred.
Essential Skills:
- Understanding of the Hospice Philosophy.
- Proficient in both oral and written communication.
- Ability to manage sensitive issues related to death and dying.
- Capability to work independently.
- Effective communication skills with facility staff when necessary.
- Familiarity with handheld communication devices and electronic documentation.
- Empathy for the needs of the ill, injured, frail, and impaired.
Key Responsibilities:
- Addresses personal care needs by responding promptly to patient and family requests, including assistance with bedpans and urinals, bathing, oral hygiene, and changing bed linens.
- Ensures patient safety and utilizes equipment properly (e.g., foot stools, side rails, oxygen).
- Plans and prepares nutritious meals, including shopping as required.
- Records vital signs such as temperatures, pulse, respiration, and blood pressure as instructed.
- Monitors and cares for the patient's skin to prevent tissue breakdown.
- Reports any significant changes in the patient's condition to the supervising nurse.
- Assists with ambulation and exercises as directed by the primary care nurse or therapist.
- Provides light laundry assistance and maintains a clean living environment for the patient.
- Performs range of motion exercises and other simple procedures as directed.
- Offers respite for the patient’s family or caregiver when present.
