We are seeking a skilled Healthcare Navigator Coordinator to join our team at Thomas Jefferson University & Jefferson Health.

Job Summary

This is a critical role that requires a strong passion for patient care, excellent communication skills, and a deep understanding of healthcare systems. The successful candidate will work closely with patients, families, and healthcare providers to ensure seamless care coordination throughout their treatment journey.

Key Responsibilities
  • Serves as a key point of contact for patients, advocating on their behalf and providing emotional support
  • Promotes timely access to necessary care services, including appointment scheduling and record management
  • Maintains accurate records of patient interactions and care plans, ensuring effective communication among healthcare teams
  • Cultivates relationships with community resources, facilitating connections to support patient health and well-being
  • Ensures compliance with established care plans, addressing barriers to care and documenting all interactions in the electronic medical record
Requirements

To succeed in this role, you will need:

  • A high school diploma or equivalent required; a bachelor's degree preferred
  • At least two years of experience in clinical or community resource settings, with a focus on care coordination
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Strong organizational and time management skills, with the ability to prioritize multiple tasks and meet deadlines
  • A service-oriented approach, with a commitment to supporting patients and their families
What We Offer
