At Jefferson Health, we are seeking a highly skilled Patient Care Navigator Lead to join our team of healthcare professionals.

Job Summary

The Patient Care Navigator Lead will be responsible for providing navigation and support services to patients undergoing lung cancer screening in the outpatient setting. This includes assisting with scheduling, shared decision-making, and education for patients, as well as reviewing test results and coordinating additional workup as needed.

Responsibilities
  • Interact with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Assist with scheduling, shared decision-making, and education for patients undergoing lung cancer screening, supervised by a CRNP.
  • Opportunity to obtain master tobacco treatment specialist (MTTS) certification. Training will be provided.
  • Review test results and coordinate additional workup for patients, including follow-up scans, diagnostic procedures, and/or evaluation by additional subspecialists in Pulmonary Medicine or in the NCI-designated Sidney Kimmel Cancer Center.
  • Record results and track patients who are due for follow-up in the Lung Cancer Screening Program using state-of-the-art radiology tracking software. Training will be provided.
  • Lead community outreach efforts together with support organizations to provide education and increase enrollment in lung cancer screening.
  • Participation and collaboration on clinical research projects focused on early detection of lung cancer will also be expected as a minor part of this role.
Requirements
  • Current PA RN License and current BLS certification is required.
  • Two (2) to three (3) years of clinical experience in a patient care setting is required.
Compensation and Benefits

We offer a competitive salary range of $65,000 - $90,000 per year, depending on experience, as well as a comprehensive benefits package that includes medical, dental, and vision insurance, 401(k) matching, and paid time off.
