Job Summary:

We are looking for a skilled Speech Language Pathologist to provide rehabilitative care and skilled nursing to clients in their homes. As a member of our multidisciplinary health care team, you will make home visits to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.

Responsibilities:
  • Conduct thorough evaluations to assess client needs and develop individualized treatment plans.
  • Provide speech therapy services to clients in their homes, including education and instruction on rehabilitative care and activities.
  • Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
  • Collaborate with other healthcare professionals to ensure comprehensive care and services.
Qualifications:
  • A current license as a Speech Language Pathologist in Ohio.
  • Possess a certificate of clinical competence in speech from the American Speech and Hearing Association, or equivalent educational requirements and work experience.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
