Job Title: Home Health Licensed Practical Nurse LPN Full Time

Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Health LPN. As an LPN with Aveanna, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver exceptional patient outcomes.

Responsibilities:

  • Deliver quality patient care in compliance with physician orders under the direction of a Registered Nurse teammate.
  • Perform the continuum of hands-on client care through documentation and timely communication with the client's physician and other caregivers.
  • Recognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk.
  • Educate clients and their family members based on client-specific needs.
  • Provide care in patients' homes using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes, etc.

Requirements:

  • An active LPN license in the state of application.
  • Valid CPR and driver's license in the state of application.

Preferred Qualifications:

  • Medicare Skilled Nursing experience.
  • Basic understanding of Oasis.
  • 1-year LPN experience in a home health or healthcare setting.
  • Home Care Home Base experience a plus.
