Position Overview: As the ALF (Assisted Living Facility) Nursing Care Supervisor, you will be responsible for overseeing and guiding the healthcare functions for residents, ensuring exceptional care quality and a supportive work atmosphere in your designated service areas.
Key Responsibilities:
- Evaluate residents' needs, coordinate personalized care plans, and cultivate positive interactions with residents, their families, and staff.
- Perform comprehensive assessments of residents' physical and mental health, as well as available family and community resources.
- Ensure that pre-admission screenings are conducted to determine the suitability of placement and readiness to address residents' care requirements.
- Collaborate with multidisciplinary teams to create, implement, review, and modify individualized care/service plans for each resident, ensuring regular assessments are conducted, especially when there are changes in condition.
- Supervise the administration of medications to residents, maintain precise medical records, and appropriately delegate nursing responsibilities.
- Engage with physicians to coordinate care for residents and liaise with external medical service providers as necessary.
- Foster strong relationships with residents, families, staff, and relevant support groups through proactive communication and constructive feedback.
- Perform additional duties as assigned.
Qualifications:
- Must be a Registered Nurse in the state of Oregon; a Bachelor of Science in Nursing (BSN) is preferred.
- At least 2 years of nursing experience, preferably in a long-term care environment.
- A minimum of 1 year of supervisory experience.
- Ability to interact effectively with staff, residents, and families.
- Strong organizational, time management, and project management skills.
- Must consent to and successfully pass a criminal background check.
