Key Responsibilities:
  • Support Client Health and Well-being: Ensure the physical and emotional well-being of our clients, providing assistance with medication, treatment plans, and daily living activities.
  • Assist with Daily Life Activities: Help clients with meal preparation, household chores, money management, and personal care, promoting independence and self-sufficiency.
  • Facilitate Community Involvement: Encourage clients to participate in community events, clubs, and activities, fostering social connections and a sense of belonging.
  • Develop Personalized Plans: Collaborate with clients to create personalized plans, setting goals and objectives that promote their overall well-being and quality of life.
  • Provide Transportation and Accompaniment: Safely transport clients to appointments, events, and activities, ensuring their comfort and safety.
  • Document Progress and Outcomes: Maintain accurate records of client progress, outcomes, and any challenges encountered, informing future support strategies.
Requirements:
  • Age Requirement: Must be at least 21 years old due to auto liability coverage.
  • Communication Skills: Effectively read, write, and communicate verbally in English, with the ability to understand instructions and perform record-keeping tasks.
  • Background Checks: Meet and maintain all requirements of applicable state agencies, including good standing with state abuse and neglect registries.
  • Computer Skills: Ability to learn basic computer skills, including data input and report generation.
  • Experience: Prior experience as a direct support worker is preferred.
Working Environment:

As a Community Access Associate, you will work in a variety of settings, both indoors and outdoors, supporting clients in their homes, communities, and other environments. You will be required to work some evenings and be able to pass a drug screen and criminal background check.
