Position Overview:

The primary objective of the Patient Navigator at Tidelands Health's Cancer Center is to guide newly diagnosed cancer patients from local communities to receive prompt, patient-focused care within the TH Cancer Center Network. The Patient Navigator will foster relationships with patients, ensuring they feel supported and informed throughout their journey. This role involves coordinating appointments, maintaining communication with the patient's entire healthcare team, and addressing specific patient needs. The Navigator will work to eliminate barriers to timely cancer treatment and enhance access to high-quality care for underserved populations. Recognizing the importance of supportive care from the moment of diagnosis, the Navigator will administer surveys to assess patients' symptom burdens, financial concerns, and overall well-being, which will inform navigation and care coordination efforts. Interventions may include providing education, appointment reminders, assistance with insurance and transportation, childcare solutions, and connecting patients with psychosocial and palliative care resources. The Navigator will also ensure seamless communication between oncology and primary care teams.

Key Responsibilities:

• Initiate contact with patients prior to their first appointment to establish rapport and provide ongoing support.

• Collaborate with patients, healthcare providers, and the TH Cancer Center team to set and document care goals.

• Address logistical challenges that may hinder patients from attending appointments, including scheduling conflicts and childcare needs.

• Assist patients with insurance inquiries by connecting them with financial resources and support teams.

• Facilitate access to necessary supportive care services, including psychosocial and palliative care.

• Educate patients about their care plans, potential side effects, and available clinical trials, and assist them in engaging with their care teams.

• Provide culturally competent services to patients from diverse backgrounds.

• Maintain consistent communication with healthcare providers through electronic medical records and case review meetings.

• Refer patients to internal or external case management services for additional support as needed.

• Advocate for patients and educate them on community-based and hospital-based programs.

• Document all interventions in the appropriate electronic medical record system.

• Build and sustain strong relationships with oncology appointment schedulers and related services.

• Collaborate with medical interpreters to assist patients who speak different languages.

• Compile mid-year and end-of-year reports on program activities, including data analysis and case studies.

• Participate in evaluation and research activities related to the program as required.

Qualifications:

Education: Associate's degree in nursing required; BSN preferred.

Experience:

  • Minimum of five years of nursing experience in a hospital setting, preferably outpatient.
  • At least two years of experience in oncology or outpatient infusion required.
  • Two years of chemotherapy experience required.
  • Prior patient navigation experience preferred.
  • Experience working with culturally diverse populations required.
  • Strong knowledge of oncology programs and community resources for oncology patients.
  • Commitment to improving care for high-risk patients.
  • Ability to establish effective working relationships within the medical community required.
  • Experience with specialty pharmacies.
Licensure/Certification:
  • Current RN license in good standing required.
  • Certification as an Oncology Nurse (OCN) required or must obtain within 18 months of hire.
  • BSN preferred.
  • Basic Life Support (BLS) certification required.
Knowledge/Skills/Abilities:
  • Ability to remain composed under pressure and maintain professionalism.
  • Adaptability to changing circumstances and priorities.
  • Ability to work independently with minimal supervision.
  • Strong communication skills, both verbal and written.
  • Proficient in basic computer skills and knowledge of Microsoft Office.
  • Ability to manage multiple responsibilities and projects effectively.
  • Familiarity with organizational policies and procedures.
Other:

- Valid driver's license and access to personal transportation for travel between work locations.

Physical Requirements:

While performing the duties of this position, the employee is frequently required to stand, sit, and walk; use hands and fingers; and communicate effectively. The employee must be able to exert 15 to 20 pounds of force occasionally and/or 5 to 10 pounds of force frequently. The physical demands described are representative of those required to successfully perform the essential functions of this job.
