Position Overview:

The primary objective of the Patient Navigator at Tidelands Health's Cancer Center is to guide newly diagnosed cancer patients from local communities towards timely, patient-focused treatment within the TH Cancer Center Network. The Patient Navigator will establish a rapport with patients, facilitate their journey to appointments, and maintain effective communication with the entire care team, ensuring that patient needs are met through clearly defined goals. This role is crucial in reducing barriers to prompt cancer care and increasing access to high-quality services for underserved populations.

Recognizing the importance of supportive care from the moment of diagnosis, the Patient Navigator will conduct supportive care surveys to assess patients' symptom burden, financial concerns, and overall well-being, thereby informing navigation and care coordination strategies. Responsibilities may include providing education, appointment reminders, assistance with insurance, transportation arrangements, childcare solutions, interpretation services, and connecting patients with psychosocial and palliative care resources.

The Patient Navigator will also screen for mental health issues and distress, ensuring timely referrals to social work and psychiatric services when necessary. This role acts as a liaison between oncology and primary care teams, fostering open communication. Additional responsibilities may include involvement in program-related activities as required.

Key Responsibilities:

  • Initiate contact with patients via phone prior to their first appointment to introduce services and provide ongoing support.
  • Collaborate with patients, healthcare providers, and the Cancer Center team to establish and document care goals.
  • Address logistical challenges that may hinder patient attendance at appointments.
  • Assist patients with insurance inquiries by connecting them with relevant financial resources.
  • Facilitate access to necessary supportive care services, including psychosocial and palliative care.
  • Educate patients about their care plans, potential side effects, and available clinical trials.
  • Deliver culturally competent services to a diverse patient population.
  • Maintain regular communication with healthcare providers through electronic medical records and case review meetings.
  • Refer patients to case management services for additional support as needed.
  • Advocate for patients and provide education on community and hospital-based programs.
  • Document all interventions in the electronic medical record accurately.
  • Develop strong working relationships with appointment schedulers and related services.
  • Utilize medical interpreters to assist patients who speak different languages.
  • Compile mid-year and annual reports on program activities, including data analysis and case examples.
  • Participate in evaluation and research activities as required.
Qualifications:
Education: Associate's degree in nursing required; BSN preferred.
Experience:
  • Minimum of five years of nursing experience in a hospital setting, with outpatient experience preferred.
  • At least two years of experience in oncology or outpatient infusion settings required.
  • Two years of chemotherapy experience required.
  • Prior patient navigation experience preferred.
  • Experience working with diverse populations required.
  • Strong knowledge of oncology programs and community resources for patients.
  • Demonstrated commitment to improving care for high-risk patients.
  • Ability to establish effective relationships within the medical community required.
  • Experience with specialty pharmacies.
Licensure/Certification:
  • Current RN license in good standing required.
  • Certification as an Oncology Nurse (OCN) required or must obtain within 18 months of hire.
  • BSN preferred.
  • Basic Life Support (BLS) certification required.
Knowledge/Skills/Abilities:
  • Ability to remain composed under pressure and maintain professionalism.
  • Adaptability to changing circumstances and priorities.
  • Ability to work independently with minimal supervision.
  • Proficient communication skills, both written and verbal.
  • Basic computer skills and familiarity with Microsoft Office.
  • Capability to manage multiple responsibilities and projects effectively.
  • Ability to utilize technology to fulfill job responsibilities.
  • Efficient resource management skills.
  • Adherence to organizational policies and procedures.
Other:

- Valid driver's license and access to personal transportation for travel between work locations.

Physical Requirements:

While performing the duties of this job, the employee is frequently required to stand, sit, and walk; use hands and fingers; and communicate effectively. The employee must occasionally exert 15 to 20 pounds of force and frequently exert 5 to 10 pounds of force. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
