About the Role

We are seeking a skilled and compassionate Home Health Nurse to join our team at AccentCare, Inc.

Key Responsibilities
  • Provide high-quality nursing care to patients in their homes, following treatment plans prescribed by physicians.
  • Conduct thorough assessments of patients' medical conditions and develop individualized care plans.
  • Administer medications, perform wound care, and provide education to patients and their families.
  • Maintain accurate and complete medical records, including documentation of patient interactions and care provided.
Requirements
  • One year of clinical experience and one year of home health experience.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of operation.
  • Valid CPR certification.
  • A reliable vehicle and a valid driver's license.
