Job Summary

Hamilton Pointe Health and Rehabilitation is seeking a skilled and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Night Shift Nurse. As a Night Shift Nurse, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare facility.

Key Responsibilities
  • Provide professional assessments and documentation of patient health, assisting with management of assigned nursing units and staff.
  • Participate in the development and implementation of patient care plans, maintaining up-to-date nursing care plans and documenting progress.
  • Make daily and periodic patient visits for monitoring and observing physical/mental condition.
  • Perform routine nursing procedures while working closely with physicians and families to maintain patient health/mental state.
  • Provide emotional support to patients and their families, handling resident and/or visitor complaints, allegations, accidents, or incidents that may occur.
  • Provide medical record documentation, completing 24-hour reports, end-of-shift reports, shift narcotic counts, and documenting the destruction of outdated/discontinued medications.
  • Maintain up-to-date patient medical records.
Requirements
  • Registered Nurse (RN) or LPN Qualifications
  • Graduate of an accredited school of nursing and possess a valid RN or LPN license in good standing according to State and Federal requirements.
  • Thorough understanding of the principles of safe effective nursing practices within the State of Indiana.
  • Basic computer knowledge and skills.
  • Passion for caring for people.
  • Personal integrity and the ability to deal tactfully with residents, family members, visitors, government agencies, and other personnel.
  • Ability to be discrete and protect the integrity of confidential information and stay within Corporate Compliance and HIPAA.
  • Knowledge on the use of personal computers, Word, and Excel, and working with office machines such as fax and copying machines.
