Job Overview

PennState Health seeks a skilled Case Manager to join its team in Hershey, Pennsylvania. This role is responsible for coordinating interdisciplinary care for select patient populations, mobilizing resources, and managing systems to respond to data analysis.

About the Role

This position requires a registered nurse with two years of experience and a current state license or multistate RN license. AHA BLS certification is also necessary prior to the end of the orientation period. Preferred qualifications include experience as a case manager, discharge planning, or insurance issues, as well as appropriate certification for specialty areas and proficiency in Microsoft Office.

Responsibilities
  • Coordinate interdisciplinary professional care for select patient populations.
  • Mobilize resources and manage systems to respond to concurrent and respective data analysis.
  • Participate in developing programs to improve clinical quality and fiscal outcomes.
