Responsibilities
This role involves coordinating patient care across multiple settings, including hospitals, clinics, and community-based programs. Your duties will include:
  • Care coordination: Working closely with patients, families, and healthcare providers to develop and implement individualized care plans.
  • Risk assessment: Identifying patients at risk of complications or readmission and implementing strategies to mitigate these risks.
  • Resource allocation: Coordinating access to resources and services to support patient care, such as home health care, rehabilitation services, and social support.

Requirements
To be successful in this role, you will need:
  • A Bachelor's degree in Nursing or related field.
  • At least 2 years of experience in case management or a related field.
  • Current licensure as a Registered Nurse (RN) in the state of employment.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
