Nurse Practitioner

We are seeking a dedicated Registered Nurse to join our team at Elite Home Health, a part of LHC Group. As a Registered Nurse, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
As a Registered Nurse with Elite Home Health, you will have the flexibility to work in a Point of Care setting, providing and directing provisions of nursing care to patients in their homes. You will work closely with other healthcare professionals to achieve and maintain continuity of patient care.
Responsibilities include:
- Conducting initial and comprehensive nursing evaluations to determine primary focus of care and developing plans of care within state-specific guidelines.
- Ensuring patients meet home health eligibility and medical necessity guidelines.
- Providing patient-centered care and building trusted nurse-patient relationships.
Requirements include:
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
