Job Summary

This role involves providing nutritional care and support to residents in long-term care facilities under the guidance of federal, state, and local regulations.

The successful candidate will have a strong background in nutrition, excellent communication skills, and a commitment to resident-centered care.

Responsibilities
  • Assess residents' nutritional needs and develop personalized meal plans
  • Monitor and document residents' nutritional status and progress
  • Provide education and counseling on nutrition and healthy eating habits
  • Collaborate with interdisciplinary teams to improve residents' overall health and well-being
Requirements
  • Bachelor's degree in Nutrition, Dietetics, or a related field
  • Registered by the Commission on Dietetic Registration
  • Hold a Certification/License in good standing within the state of practice
  • Knowledge of state survey process and compliance with nutrition care regulations (preferred)
  • Strong organizational and communication skills
