Sunbelt Staffing is seeking a skilled Nurse Practitioner to join our team in a part-time capacity. As a Home Health Nurse Practitioner, you will conduct home health risk assessments for Medicare Advantage Plan members in Anoka County, MN.
Key Responsibilities- Conduct annual home health risk assessments to evaluate prior and current medical history, medical conditions, and medication history.
- Evaluate patients' medical conditions and provide education on medications and medical conditions.
- Identify gaps in care and provide recommendations for resolution.
- Conduct point-of-care testing, including fingerstick and urine testing.
- MSN degree from an accredited school of nursing.
- Nurse Practitioner License in MN.
- Current CPR certification (BLS).
- Board Certification through AANP or ANCC.
- 1 year of current NP experience.
- Previous in-home care experience.
This is a part-time position with flexible shifts between Monday and Sunday. You will have the opportunity to create your own schedule and commit to a minimum weekly or bi-weekly hourly commitment.
