Hospice Nurse

At Northern Light Health, we are seeking a highly skilled and compassionate Registered Nurse to join our Home Health Outreach team. As a Registered Nurse, you will be responsible for providing high-quality patient care to individuals in their homes, promoting independence, and improving overall health outcomes.
Key Responsibilities:- Conduct comprehensive assessments of patients' physical, emotional, and social needs
- Develop and implement individualized care plans to address patients' needs and goals
- Provide education and support to patients and their families on disease management, medication adherence, and self-care
- Collaborate with interdisciplinary teams to coordinate care and services
- Monitor patients' progress and adjust care plans as needed
- Current licensure as a Registered Nurse in the State of Maine
- 2+ years of experience in home health or hospice care
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Basic Life Support certification
- Bachelor's degree in Nursing
- Experience with electronic medical records and other healthcare software
- Knowledge of hospice and palliative care principles and practices
