Job Title: Registered Nurse - Home Health Outreach

At Northern Light Health, we are seeking a highly skilled and compassionate Registered Nurse to join our Home Health Outreach team. As a Registered Nurse, you will be responsible for providing high-quality patient care to individuals in their homes, promoting independence, and improving overall health outcomes.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs
  • Develop and implement individualized care plans to address patients' needs and goals
  • Provide education and support to patients and their families on disease management, medication adherence, and self-care
  • Collaborate with interdisciplinary teams to coordinate care and services
  • Monitor patients' progress and adjust care plans as needed
Requirements:
  • Current licensure as a Registered Nurse in the State of Maine
  • 2+ years of experience in home health or hospice care
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic Life Support certification
Preferred Qualifications:
  • Bachelor's degree in Nursing
  • Experience with electronic medical records and other healthcare software
  • Knowledge of hospice and palliative care principles and practices
