We are seeking a highly skilled and compassionate Registered Nurse to join our Hospice Care team as a Hospice Outreach Specialist. As a key member of our team, you will be responsible for delivering high-quality patient care and providing support to patients and their families in their homes.
Key Responsibilities- Provide comprehensive and holistic assessments of patients and families to identify their unique needs and develop personalized care plans.
- Deliver high-quality patient care, including administering medications, performing treatments, and providing emotional support and education to patients and their families.
- Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure seamless care coordination and communication.
- Develop and maintain relationships with patients, families, and healthcare providers to ensure continuity of care and promote patient-centered care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and outcomes.
- Current licensure as a Registered Nurse in the State of Maine.
- 2+ years of experience in hospice care or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
- Ability to work independently and as part of a team, with a strong focus on patient-centered care and collaboration.
- Strong critical thinking and problem-solving skills, with the ability to analyze complex situations and develop effective solutions.
