Job Title: Senior Choice Transportation Coordinator

Lifescape Community Services/Community Kitchen, Inc. is seeking a highly skilled and experienced Senior Choice Transportation Coordinator to join our team.

Job Summary:

The Senior Choice Transportation Coordinator is responsible for leading the transportation program, ensuring seamless scheduling and communication with clients and staff. This role requires strong organizational and documentation skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Lead the transportation program, ensuring timely and accurate scheduling and communication with clients and staff.
  • Manage and maintain the transportation database, ensuring up-to-date information and accurate documentation.
  • Provide backup support for Senior Choice caseworkers as needed.
  • Coordinate transportation trips with local community sites and Senior buildings.
  • Accurately complete and process referrals for clients to other agencies and private individuals.
  • Provide accurate information and assistance to walk-in or telephone consumers.
  • Stay up-to-date and informed on available services and how to access those services for consumers.
  • Accurately and timely complete various forms and applications for consumers with information provided by the consumer.
  • Perform home visits to assess consumer needs, identify available assistance, and complete necessary forms, phone calls, etc.
  • Inform and assist clients in gaining access to available benefits and services, including acting as a representative at appropriate agencies with consumer consent.
  • Provide timely, accurate, and ongoing follow-up with consumers to ensure their needs continue to be met.
  • Work with staff, other agencies, and private individuals to find hard-to-reach clients.
  • Maintain timely and accurate required documentation of service given to clients.
  • Assess consumer needs for services not specifically funded and submit timely and accurate requests for Gap Filling services.
  • Participate in activities and events to promote agency services.
  • Participate in Information and Assistance activities, which may include giving presentations at various locations on benefits and services.
  • Attend training as required by Supervisor or Executive Director.
  • Represent the agency professionally in manner, speech, and dress.
  • Drive the transportation van if and when needed.
Essential Functions:
  • Must be able to read, write, and understand written and verbal instructions.
  • Must be able to operate fax, copy machine, basic office machines, and multiple telephone lines.
  • Must be computer literate.
  • Must be able to effectively and patiently communicate with consumers, caregivers, staff, and visitors.
  • Must be able to interpret consumers or caregivers' needs effectively.
  • Must be able to concisely and accurately convey those needs to the appropriate agencies.
  • Must be able to interpret a variety of instructions furnished in written or oral form from government, benefit, and funding organizations.
  • Must be able to travel to and safely enter homes and other locations to perform job-related duties outside of the office environment.
  • Must be able to work independently.
Minimum Requirements:
  • Must be 21 years of age.
  • Bachelor's degree or equivalent experience preferred.
  • Must have two years of direct service experience and/or equivalent training.
  • Must have a valid IL driver's license, clean driving record, and own transportation with current insurance coverage.
  • Must not have been convicted within 3 years of the date of application of any of the following offenses involving the operation of a motor vehicle: Driving Under the Influence of alcohol and/or drugs (DUI), Reckless Driving, Leaving the Scene of an accident involving death or personal injury, Drag Racing, Manslaughter, Reckless Homicide, Reckless Conduct.
  • Must be able to work and communicate effectively with older persons, persons with sight and hearing impairments, and persons with a variety of economic, social, and cultural backgrounds with an emphasis on seniors.
  • Must be able to perform essential functions of the position.
