Job Summary:

We are seeking a skilled Licensed Practical Nurse to join our team as a Referral Management Specialist. In this role, you will be responsible for managing referrals, coordinating with healthcare providers, and ensuring seamless patient care.

Key Responsibilities:

  • Manage referrals from initial specialty care to network primary care
  • Coordinate with specialty referral clinics and track referrals
  • Provide patient education and support throughout the referral process
  • Collaborate with healthcare providers to ensure accurate and timely referrals
  • Identify cases that may benefit from alternative levels of care and involve case managers as needed

Requirements:

  • Current LPN licensure in the United States
  • 1 year of experience in nursing, preferably in utilization management
  • Operational computer skills and knowledge of medical care criteria
  • Excellent communication and customer service skills
