  • Collaborate with Healthcare Providers: Work closely with healthcare providers, including assigned clinic physicians, specialists, and onsite clinic pharmacists, to ensure seamless medication management and coordination of care.
  • Perform Clinical Assessments/Educational Counseling: Conduct clinical assessments and educational counseling at the drug level for all patients within high-priority clinics on qualified medications.
  • Ensure Clinical Compliance: Ensure that all clinical criteria are met in compliance with FDA, clinical guidelines, and evidence-based literature.
  • Assist with Workflow Optimization: Identify areas for improvement and implement strategies to enhance efficiency, productivity, and quality of care.
  • Advocate for Patient Safety and Medication Literacy: Promote patient safety and medication literacy through patient education and counseling.
  • Protect Confidentiality: Maintain the confidentiality of patient information.
  • Participate in Professional Development: Attend and participate in team meetings, conferences, and other professional development activities to stay current on clinical practices and emerging trends.

Requirements:

  • Education: Doctor of Pharmacy (PharmD) degree.
  • Licensure/Certification: Current licensure as a pharmacist in the state of employment.
  • Experience: 1+ years of experience as a clinical pharmacist, preferably in specialty pharmacy.
  • Skills: Strong analytical and problem-solving skills, excellent verbal communication skills, and deep knowledge of pharmacology developments and FDA health regulations.
  • Traits: Organized, high-integrity, attention to detail, dependable, quality focus, empathetic, and a good listener/communicator.
