Position Overview:


Under the general oversight of the OB/GYN Chief of Service for clinical matters and the Department Administrator for administrative concerns, the Lead Certified Nurse Midwife operates as a pivotal member of the healthcare team.

This role involves coordinating departmental functions to guarantee high-quality patient care within a secure and nurturing environment that prioritizes both patients and their families. The Lead CNM is accountable for their professional practice and serves as an expert resource with the necessary clinical skills. They provide support to team members while being attuned to departmental priorities and needs. This position exemplifies professional Nurse-Midwifery practice and may suggest efficient resource allocation to management by ensuring proper scheduling and coverage. The Lead CNM's performance aligns with the strategic objectives of the organization and the mission, vision, and values of Kaiser Permanente.

In collaboration with an Ob-Gyn Physician, the Lead CNM oversees the medical care of patients throughout the maternity cycle, which includes antepartum, intrapartum, postpartum, and gynecological care.

They deliver standard maternity care and guidance to expectant mothers, adhering to established ambulatory care and inpatient protocols, policies, and procedures as mandated by the State of California Nurse Practice Act and Southern California Permanente Medical Group.


Key Responsibilities:


The Lead Certified Nurse Midwife showcases their expertise through the following skills, competencies, and behaviors: Leadership: Oversees, reviews, assigns, and organizes the tasks of at least three (3) or more Certified Nurse-Midwives.

They apply the KP Nursing mission, vision, values, and behaviors in their daily responsibilities. Understanding departmental needs and setting priorities is essential. They provide professional leadership and direction to Certified Nurse-Midwives to ensure the effective delivery of patient care. This role includes coordinating departmental operations and appropriately delegating tasks. The Lead CNM serves as an exemplary role model for professional Midwifery practice (e.g., CNM/MD Collaborating for Outcomes, Just Culture, Quality, Service, & Safety). They support and enhance the leadership capabilities of staff RNs, LVNs, MAs/CNAs, and other ancillary personnel as necessary.

Compliance with Kaiser Permanente's Policies & Procedures, Principles of Responsibilities, and applicable State & Federal regulations is paramount.

They uphold the standards set forth by the California Nurse Practice Act, the American Nurses Association (ANA) Standards of Nursing Practice, the ANA Code of Ethics for Nurses, and the American College of Nurse-Midwives.

The Lead CNM champions innovative ideas and actively collaborates with others throughout the change process.

They act as a resource, preceptor, and mentor to new employees, residents, registry, students, and other team members regarding department-specific operations and patient care activities.

Collaboration with or referral to the manager/clinical nurse specialist/educator is necessary for any outstanding issues or concerns within the unit. The Lead CNM demonstrates the ability to identify and resolve problems in partnership with work teams across the continuum. They ensure adherence to regulatory requirements, policies, procedures, and standards of practice. Participation in activities related to patient flow, including admission and discharge processes, is expected. They facilitate inter-professional/interdisciplinary communication and collaboration.

Nursing Process:

Serves as a resource for clinical inquiries, scope of practice, and applicable State & Federal regulations, as well as operational concerns of providers and staff.

Possesses the experience and skills to be a resource for providers, RNs, LVNs, MAs, CNAs, and other ancillary staff as appropriate.

Flexibility in work schedule to meet departmental needs is essential. Active involvement in assigning, orienting, precepting, and coaching staff and students as required is expected.

Documentation:

Conducts audits and ensures that documentation accurately reflects thorough assessments, incorporating physical and psychosocial findings, diagnostic tests, learning needs, barriers, multidisciplinary education plans, and age-appropriate care.


Clinical Outcomes:

Participates in and leads various aspects of departmental operations, staff competencies, departmental committees, Unit Based Teams (UBTs), new projects, quality initiatives, and educational programs.

Demonstrates knowledge of patient-specific conditions commonly encountered in the department and the necessary care required. Engages with leadership, peers, physicians, other healthcare providers, and support staff regarding performance as needed.

Actively participates, champions, and motivates other staff in quality improvement, patient care, workflow, and enhancements in clinical initiatives and working conditions within the department.

Provides input on employee performance evaluations through provider and patient feedback, peer review, and chart review.

Workplace Safety:

Adjusts staffing requirements of the department to ensure quality patient care, patient and staff safety, efficiency, productivity, and favorable working conditions for providers and staff.

Actively models Labor-Management Partnership (LMP) Workplace Safety principles and practices.

Patient Care Experience/Customer Service:
Acts as a role model for staff in delivering a patient care experience that surpasses member expectations. Ensures timely and appropriate management of patients. Participates in a seamless transition for patients across the care continuum. May engage in daily leadership rounds and document rounding activities as appropriate.

Team Commitment:
Ensures support for the collaborative LMP environment through unit-based teams (UBTs). Identifies and supports staff professional/clinical development needs (i.e., certification, presentations, reward/recognition). Motivates staff to reach their fullest potential in enhancing performance, attendance, access, and service quality outcomes. Assures consistent communication of departmental meeting information. Approaches difficult or conflict situations constructively and seeks appropriate assistance.

Fiscal Responsibility:
Participates in resource allocation (assignments, scheduling, and certain equipment/supply management).

Collaborates and assists with leadership, physicians, and peers in scheduling, workflow, and throughput to promote quality patient care, patient and staff safety, efficiency, productivity, and favorable working conditions for providers and staff.


Professional Development:
Attends Professional Development Educational programs. Takes responsibility for ongoing professional growth and development.

In addition to the Certified Nurse Midwife Job Details, the Lead CNM is expected to demonstrate proficiency by exhibiting the following skills, competencies, and behaviors:

Under the supervision of an Ob-Gyn physician, manages the care of women during labor and delivery.

Obtains a comprehensive medical, obstetrical, gynecological, and psycho-social history of the current pregnancy.

Observes indicators of the member's general and obstetrical health status.

Conducts thorough physical examinations, including pap smears, breast examinations, abdominal examinations, pelvic examinations, and clinical pelvimetry.

Records all findings from examinations, histories, and significant observations in the patient's chart.

Determines pregnancy status during gestation, presentation, and position of the fetus, as well as fetal heart rate.

Assesses whether all findings are within normal limits.

Orders necessary laboratory examinations such as chest X-rays, ultrasounds, CBC, urinalysis, blood type and Rh factor, and serology tests.


Consults with and obtains orders from an obstetrician for any special care or diagnostic procedures and refers to other services as necessary.

Implements preventative measures and identifies abnormal conditions, seeking consultation and assistance from physicians as needed.

Educates, counsels, and guides individual members regarding their specific healthcare needs.

Identifies the need for social services, genetics, or mental health services and refers as appropriate.

Evaluates patients' general and obstetrical health status.

Provides health supervision, care, and family planning advice to women during antepartum and postpartum periods.


Assesses fetal status by performing procedures such as abdominal palpation and ultrasound examinations to estimate fetal weight, presentation, position, approximate station, and fetal heart rate.

Able to perform internal and external fetal monitoring and interpretation.

Consults with obstetricians regarding any questionable findings.

Determines the member's stage of labor and whether the member should be admitted to labor and delivery.

Independently conducts normal spontaneous vaginal deliveries.

First assists at Cesarean Sections and gynecological surgeries.

Provides immediate care for the newborn.

Basic Qualifications:
Experience

Minimum three (3) years of full-time recent experience as a CNM required.

Education

Graduate of an accredited RN program.

Master's Science Degree required in 2010.

Graduate of a nurse midwifery program accredited by the A.C.N.M. BS or BSN or BA required, MS Degree required in 2010.
