Position Overview:


We are seeking a Nurse Practitioner Mentor for our Division Wide AIM (Advanced Illness Management) Program to join our dedicated team.

This role will report directly to the Vice President of Palliative Clinical and is tasked with delivering high-quality training and interactions with both newly hired and experienced Nurse Practitioners during palliative care/AIM visits throughout the division.

The mentor will facilitate a comprehensive learning experience, ensuring preceptees can competently fulfill their responsibilities through effective mentoring, coaching, and modeling appropriate behaviors.

Home visits to patients will be conducted as necessary.


The mentor will orient new or train existing Nurse Practitioners (preceptees) on clinical policies and procedures, documentation standards, required patient outcomes, technology usage, and compliance with regulatory mandates.

Ongoing performance metrics will be monitored to provide continuous coaching and feedback to preceptees.

Experience in hospice and/or palliative care is preferred.

This position requires 75% travel monthly, focusing on specific regional coverage, and mandates licensure in the relevant states.

Candidate Profile:
Advanced Practice Practitioner credentials are required.

Unencumbered Nurse Practitioner licensure in assigned work states.

Active DEA registration.

Current CPR certification.

A minimum of 5 years of clinical experience is essential.

Familiarity with part B documentation requirements is expected.
