Key Responsibilities:
In alignment with Unified Community Connections' Mission Statement, the Admissions Specialist will:

  • Screen and monitor referrals by collecting information, scheduling, and participating in tours.
  • Report any changes in the medical status of individuals.
  • Maintain comprehensive admission records and ensure they are accurate and up-to-date.
  • Conduct daily correspondence via email and phone regarding Unified Services, families, and CCS.
  • Coordinate all adult services in compliance with relevant State and Federal regulations.
  • Track and oversee participant funding, enrollment, referrals, and admissions, providing monthly updates to the supervisor.
  • Develop and manage regional respite programs.
  • Act as a resource for the Agency and the public regarding services for individuals with disabilities.
  • Represent the Agency at provider fairs, meetings, and conferences.
  • Exhibit flexibility to serve at all Agency events during weekends or evenings as required.
  • Travel from the main office to other locations for meetings, fairs, and conferences as necessary.
  • Adhere to Unified Community Connections policies and procedures.
  • Ensure all established deadlines are met.
  • Follow the Attendance and Punctuality policy as outlined in the Employee Handbook.
  • Perform other duties as assigned.


Qualifications:
The following qualifications are necessary for consideration:

  • Bachelor's Degree in Social Work, Psychology, or related Human Services fields is preferred. A High School Diploma or GED with relevant knowledge of the disability sector will also be considered.
  • Two (2) to five (5) years of experience in a Residential or Personal Support Services environment.
  • Completion and maintenance of all required DDA training.
  • COVID-19 vaccination.
  • Valid Driver's License with a Motor Vehicle Record showing 2 or fewer points.
  • Ability to lift 50 lbs. unassisted.


Skills:
Successful candidates will demonstrate:

  • Strong organizational skills and meticulous attention to detail.
  • Ability to read and comprehend simple instructions and written correspondence, as well as interpret documents.
  • Capability to write reports and complete necessary forms or documents.
  • Effective communication skills with individuals in Unified programs, family members, medical professionals, co-workers, and vendors, both verbally and in writing.
  • Proficiency in basic computer skills, including Outlook Email, Microsoft Word, and Excel.


Work Schedule:
Primarily Monday through Friday during daytime hours, with occasional evening and weekend commitments for Agency events as assigned.
