About the Role

We are seeking a skilled Licensed Clinical Social Worker to join our team at Amergis. As a key member of our multi-disciplinary team, you will provide essential social work intervention and counseling services to patients and clients in need.

Key Responsibilities
  • Assess patient psychosocial needs and develop plans to meet established goals
  • Collaborate with healthcare teams to plan and evaluate patient care
  • Provide counseling and crisis intervention services to individuals and families
  • Facilitate referrals to community agencies and resources as needed
  • Work with government agencies to assist clients with Medicare and Medicaid applications
Requirements
  • Master's degree in Social Work from a Council on Social Work Education-accredited program
  • Active LCSW license in the state of practice
  • Minimum one year of social work experience preferred
  • Strong communication and interpersonal skills
  • Computer proficiency and ability to work in a fast-paced environment
